By signing, I authorize Manzanita Creek to charge my credit card at the time of each shipment. I understand that membership is ongoing and may be cancelled at any time after one year of membership, or receiving 2 shipments, and that there is a $50 cancellation fee should I cancel before that time. I agree that if I opt to pick up my wine, I will do so within 60 days of release or wine will be shipped to the address on file and shipping charges will apply. I declare that I am at least 21 years of age. I understand that any changes in contact information, shipping address or credit card information should be conveyed immediately to avoid delays in shipping or additional shipping charges for returned shipments.
